The Chronic Pain Clinic at GV Health is seeking a Grade 1 Dietitian to join the clinic on a full time or part time basis until June 2020.

The Dietitian will work as an active member of three multidisciplinary teams within the subacute specialist clinics. This will include the Chronic Pain Clinic (CPC), the Cognitive Dementia and Memory Service (CDAMS) and the Continence Clinic.

The dietitian will provide a comprehensive service to clients across the three services which will include the planning, implementation and coordination of specific dietetics programs for individual patients and groups. The dietitian will have a strong focus on continuous improvement, and the ongoing evaluation of the quality and outcomes of dietetic interventions.

The CPC provides clinical assessment and where indicated, provision of treatment on a time-limited basis, for clients experiencing prolonged pain and moderate disability and for those who have entrenched chronic pain and high levels of disability. The aim is to reduce the risk of long-term disability, enable clients to gain an improved ability to self-manage their condition and develop strong links to support continuing management in a primary care setting following discharge from a specialist service.

The Cognitive Dementia and Memory Service (CDAMS) is a specialist multidisciplinary diagnostic, referral and educational service for people experiencing memory loss, or changes to their thinking (cognition) and those who care for them. CDAMS provides home or clinic based multidisciplinary assessments for expert clinical diagnosis, and links to appropriate services; however the service does not provide ongoing treatment or case management.

The Continence Clinic aims to improve and maintain a person’s continence, functional capacity and maximise their independence.

The role requires you to possess excellent communication skills, be well organised and have a strong focus on patient centred care.
